1. Overview
HealthPro Matcher (healthpromatcher.com) is owned and operated by Marketcall Inc. ("we," "us," or "our"). HealthPro Matcher is a free matching service that connects consumers with independent dental and rehabilitation providers. We are not a dental office, treatment center, or medical provider, and we do not provide medical, dental, or clinical advice.
This page applies solely to California residents and describes the additional rights available to you under the CCPA/CPRA, together with how to submit a request. We do not maintain consumer accounts on this website.
2. Categories of Personal Information We Collect
In the preceding 12 months, we have collected the following categories of personal information from California consumers:
- Identifiers and contact details: such as your name, email address, phone number, ZIP code, and IP address;
- Service-request details: the information you provide about the care you are seeking, such as the type of dental or rehabilitation service you are interested in and your general preferences;
- Internet or other electronic network activity: such as pages viewed on our site and interactions with our web forms;
- Geolocation data: approximate location derived from your ZIP code or IP address, used to identify providers near you; and
- Inferences: limited inferences drawn from the information above to match you with providers who may be able to help.
We do not collect credit reports, credit scores, or financial account information, and we do not perform credit checks of any kind. We collect the information above directly from you when you submit a form on our website, and automatically through cookies and similar technologies as described in our Cookie Policy.
3. How We Use and Disclose Personal Information
We use the categories of personal information described above for the following business and commercial purposes:
- Matching you with independent dental and rehabilitation providers relevant to your request;
- Routing your request to the providers who can help and, where you have consented, contacting you about it;
- Communicating with you by phone, email, or SMS regarding dental and rehabilitation offers;
- Operating, maintaining, analyzing, and improving our website and service;
- Detecting and preventing fraud and abuse; and
- Complying with applicable legal obligations.
We disclose your contact details and service-request information to the independent providers who can address your request so they may contact you. We may also disclose information to service providers that support our operations (such as hosting, analytics, and communications vendors) and as otherwise described in our Privacy Policy. We do not disclose your information to credit bureaus or for credit-related purposes.
4. Your Rights Under CCPA/CPRA
Subject to certain exceptions, California residents have the following rights:
Right to Know and Access
You have the right to request that we disclose the categories and specific pieces of personal information we have collected about you, the categories of sources, the business or commercial purposes for collecting or sharing it, and the categories of third parties to whom we disclosed it.
Right to Delete
You have the right to request that we delete personal information we have collected from you, subject to exceptions permitted by law (for example, where retention is necessary to complete a request you made, detect security incidents, or comply with a legal obligation).
Right to Correct
You have the right to request that we correct inaccurate personal information we maintain about you, taking into account the nature of the information and the purposes for processing it.
Right to Opt Out of Sale or Sharing
You have the right to direct us not to "sell" or "share" your personal information as those terms are defined under the CCPA/CPRA. See Section 5 below for how to exercise this right.
Right to Non-Discrimination
We will not discriminate against you for exercising any of your CCPA/CPRA rights. We will not deny you our service, charge you a different price, or provide you a different level or quality of service because you exercised your rights.
5. Do Not Sell or Share My Personal Information
When you submit a request through our website, we disclose the contact details and service-request information you provide to the independent providers who can help you, and we may share certain identifiers and online activity with advertising and analytics partners for cross-context behavioral advertising. Under the CCPA/CPRA, some of these disclosures may be considered a "sale" or "sharing" of personal information.
To opt out, please email info@healthpromatcher.com with the subject line "Do Not Sell or Share My Personal Information." Include your full name and the email address you used when submitting a request on our site. We will process your opt-out request within 15 business days.
Opting out applies on a going-forward basis and does not affect information already disclosed prior to your request. If you later submit a new request on our website, we will treat that submission as a new instruction to share your information for the purpose of matching you with providers, unless you indicate otherwise. We do not knowingly sell or share the personal information of consumers under 16 years of age.
6. How to Submit a Request
To submit a verifiable consumer request to know, access, delete, or correct your personal information, email info@healthpromatcher.com with the subject line "California Privacy Rights Request." Please describe the right you wish to exercise and include your full name, email address, and phone number so we can verify your identity.
We will acknowledge receipt of your request within 10 business days and respond to verifiable requests within 45 calendar days. If we need more time (up to a total of 90 days), we will notify you in writing. To protect your privacy, we will verify your identity — typically by confirming the email address you provide — before processing your request. We do not charge a fee for processing verifiable requests unless they are excessive or repetitive.
7. Authorized Agents
You may designate an authorized agent to submit a CCPA/CPRA request on your behalf. Your agent must provide written proof of authorization signed by you, or a valid power of attorney executed in accordance with California Probate Code sections 4000 to 4465. We may still contact you directly to verify your identity. Authorized agents may submit requests by emailing info@healthpromatcher.com with proof of authorization attached.
